Digitise 40,000 paper patient records

Project Description

Six years of paper case sheets from four of our older clinics. Handwritten, in English with abbreviations, of varying legibility. We need them into a defined schema with a confidence flag where the transcription is uncertain, so our doctors know when to pull the original. A wrong drug name is worse than a blank field. This is health data. We will sign an agreement and the work happens on machines we can account for.
